I was introduced to Matt Urban (The Matt Urban Human Services Center of WNY) through Will Marcy and his outreach. One of the many services this local not-for-profit organization provides is a Homeless Outreach, connecting the shelterless with housing and medical care. The Homeless Outreach team (which includes a nurse, case manager and social worker) goes out twice a week in collaboration with UB Heals.
